Passports for Kids

Under 16: Child and both parents must come.
Bring proof of parent/guardian relationship (birth certificate, court papers, adoption papers).
If one parent can’t come:
- Notarized consent form (DS-3053) + copy of parent’s ID
- Court order for sole custody
- Birth certificate with one parent listed

Ages 16–17:

Must show parent knows about the application (parent can come, pay, or confirm ID).
